The NC Rural Health Transformation Program for Medicaid Region 6, led by Access East is conducting a survey across 26 counties in eastern North Carolina to better understand current strengths and needs related to health care.

This survey is for anyone who lives, works, or receives care in eastern North Carolina. You do not need insurance or a specific diagnosis to participate. 

This survey takes about 12 minutes, and responses are completely anonymous.
